TMJ Disorder

Understanding
TMJ Disorder

TMD stems from abnormal jaw actions, often linked to undiagnosed sleep breathing issues. Dr. Krish offers comprehensive treatment addressing the underlying causes of your pain.

What Is TMJ Disorder?

Temporomandibular disorder, commonly known as TMD, is a condition that stems from abnormal jaw actions and dysfunction of the temporomandibular joint. The TMJ connects the jawbone to the skull and allows for essential movements like chewing, speaking, and yawning.

TMD is frequently linked to undiagnosed sleep breathing issues. Patients often suffer from a range of symptoms including headaches, jaw pain, facial pain, neck and back pain, jaw popping and clicking, difficulty opening the mouth, catching or locking of the jaw, and ear pain or ringing.

TMD Symptoms & Headaches

Many patients with TMD experience chronic headaches that are often misdiagnosed as migraines or tension headaches. These primary headaches can be debilitating and are frequently a direct result of jaw dysfunction and muscle tension.

Additional symptoms include facial, neck, and back pain that radiates from the jaw area, as well as jaw popping, clicking, and difficulty opening the mouth fully. Ear pain and ringing (tinnitus) are also common complaints.

TMJ is short for temporomandibular joint. You have two of these joints, located just in front of your ears on either side of your head. They act like sliding hinges, working together as a pair to connect your lower jaw to your skull so you can open and close your mouth.

TMD can result from major physical injuries or repetitive stress over time:

  • Macrotrauma: Direct impact or injury to the jaw or joint, such as from an accident, fall, or blow to the face.
  • Microtrauma: Gradual wear and tear or repetitive strain caused by:
    • Chronic teeth clenching and grinding (bruxism), especially during sleep
    • Inflammatory or systemic conditions like osteoarthritis, rheumatoid arthritis, fibromyalgia, or Ehlers-Danlos syndrome
    • Complications from infections that spread inflammation to the jaw joint (such as dental abscesses or infectious arthritis)

Common symptoms of TMD include:

  • Morning discomfort: Waking up with pain or tenderness in your jaw, face, or a morning headache.
  • Jaw locking: Waking up with your jaw feeling stuck or unable to open wide.
  • Joint noises: Clicking, popping, or grating sounds when eating or opening and closing your mouth.
  • Ear pain: A persistent ache in or around the ears.
  • Discomfort when opening wide: Pain when stretching your mouth open, such as during a dental visit or while eating large foods.

Yes. TMJ disorder is one of the most common causes of chronic headaches and migraines. The trigeminal nerve, which is responsible for most head and face sensation, runs directly through the jaw joint area. When the TMJ is inflamed or misaligned, it can trigger frequent, severe headaches that are often misdiagnosed as tension headaches or migraines.

Most jaw pain improves with conservative care. Here is how you can begin managing it at home:

  • Rest your jaw: Choose soft foods (like smoothies, soups, and eggs) and stop chewing gum or hard items.
  • Apply temperature therapy: Use a moist heat compress to relax tight muscles, or an ice pack to numb acute pain.
  • Manage inflammation: Over-the-counter anti-inflammatories, such as ibuprofen, can offer short-term relief.
  • Perform light stretches: Gently open and close your mouth to keep the muscles limber without pushing past your comfort zone.

Next Steps:
If your symptoms persist, worsen, or interfere with your daily life, contact our office for a professional assessment. We can help identify the root cause and create a tailored treatment plan for you.

Effective TMD treatment starts with pinpointing the exact cause of your discomfort. We take a precise, tech-forward approach to your diagnostic care:

  • Detailed Review: We examine your full dental and medical history to connect all the dots.
  • Targeted Physical Exam: We gently palpate the muscles in your face, jaw, neck, and shoulders to identify trigger points.
  • 3D Imaging (CBCT): A fast 3D scan provides an in-depth view of your jaw joints, surrounding bone structure, and airway.
  • Joint Vibration Analysis (JVA): We measure invisible joint friction and vibrations to evaluate how smoothly your jaw functions.
  • Mess-Free Digital Impressions: We replace traditional, uncomfortable putty with precise digital 3D scans. This guarantees exceptional accuracy when designing your custom orthotics for maximum comfort and relief.

We offer several non-surgical treatment options including custom oral appliances to realign the jaw, trigger point injections for immediate pain relief, cold laser therapy to reduce inflammation, SPG nerve blocks for chronic pain, myofunctional therapy to strengthen supporting muscles, and PRF injections to promote healing. Dr. Krish will recommend the best combination based on your specific diagnosis.

TMJ orthotics—often referred to as oral appliances, mouthguards, or splints—are custom-crafted devices that fit comfortably over your teeth.

How they help:

  • Stabilize the Joint: They gently hold your jaw in its ideal, stress-free position so your joints and muscles can rest and align properly.
  • Relieve Muscle Strain: By balancing your bite, they reduce tension throughout your jaw, face, neck, and shoulder muscles.
  • Calm Inflammation: Easing joint strain decreases swelling and tissue irritation, allowing your jaw to heal.
  • Restore Normal Function: Reducing pain and spasms restores your ability to chew, speak, and open your mouth comfortably.

Unlike generic, store-bought mouthguards—or traditional “nightguards” that simply protect teeth without addressing joint position—our office uses custom-engineered therapeutic orthotics designed to treat TMJ disorder directly.

Depending on your diagnostic results, we may prescribe:

  • Nighttime Orthotics: Worn while sleeping or napping, these custom devices keep your jaw properly positioned overnight. They reduce clenching and grinding, easing morning pain and improving overall sleep quality.
  • Daytime Orthotics: Worn during waking hours (typically for about 3 months), these appliances provide continuous joint decompression and stabilization, keeping the healing process moving forward throughout the day.

Yes, we accept most major medical insurance plans. TMJ disorder treatment is typically covered under medical insurance, not dental. Our team will verify your benefits and explain your coverage before treatment begins so there are no surprises.

Unlike over-the-counter “boil-and-bite” guards—which are bulky and easily shift—our custom sports guards are digitally crafted for a precise, comfortable fit.

Research shows that custom performance guards do more than just protect your teeth from trauma; they can actually boost your athletic output:

  • Maximized Protection: Absorbs and redistributes direct impact forces to safeguard teeth and jaw joints.
  • Increased Power Output: Aligning your jaw in its natural, relaxed position reduces strain and can improve head/neck stability for better power generation.
  • Unrestricted Airflow: Fits snugly so you can speak, drink water, and breathe effortlessly without gagging or adjusting your guard mid-game.

Protect your teeth and gain a competitive edge. Contact our office to book your sports guard evaluation!

TMJ & Sleep Apnea

Many TMJ patients also have undiagnosed sleep apnea. The jaw position that contributes to TMJ disorder can also restrict your airway during sleep.
